Registration Information

PROCEDURES FOR REGISTRATION AND HOTEL RESERVATION

1. Registration of participants can be done online at website www.isuog12manado.com or by filling out the registration form completely and sending it to :

2.Hotel accommodation bookings can only be made by contacting Yanti / Titin at no. 021 – 392 8721 / 0812 8286 8612, Fax. No. 021 – 392 8721

3. After registering as a participant and or after booking the hotel, please make payment via bank transfer and send payment transfer receipt to the Secretariat ISUOG Approved          Course XII through fax at no. 021 – 392 8721 or email: titin.4b4@gmail.com

4. Participants who have submitted proof of payment transfer will get a confirmation and validation email from the Committee ISUOG Approved Course XII

5  Proof of registration or validation must be brought by the participant at the time of re-registration.

INSTRUCTIONS CANCELLATION OF PARTICIPANTS

1. The Cancellation of the participants must be submitted in writting by letter or email addressed to the Secretariat of isuog Approved Course by giving the reasons for the cancellation of participation.

2. Submission of cancellation can be summited not later than 1 ( one ) month before the event.

3. The committee reserves the right to refuse to give refund if the reasons given are not clear and sufficient.

4.  Participants who submit a cancellation are expected to replace them with the names of the other participants.

INSTRUCTIONS TO CHANGE OR CANCELLATION OF HOTEL BOOKING

  1. If for any reason, the reservation of the hotel accommodation will be cancelled, all payments made will be forfeited. The committee did not provide refunds.
  2. Payment must have been received by the Committee no later than 2 weeks prior to the order date.
  3. Changes to the date and name of the hotel accommodation reservation must be made no later than 2 weeks before the date of arrival. For  the reservation changing in less than 2 weeks from the date of arrival, the payment already made will be forfeited Changes in the date of booking can be made based on room availability
